Mechanism

BPC-157 is a synthetic 15-amino-acid peptide corresponding to a partial sequence of a protein (BPC) isolated from human gastric juice. In preclinical models its effects are attributed largely to modulation of the nitric oxide (NO) system — interacting with the L-arginine–NO pathway and NO synthase activity — and to promotion of angiogenesis through upregulation of VEGFR2 and downstream activation of the VEGFR2–Akt–eNOS and FAK–paxillin pathways, which drive endothelial-cell migration and tube formation. It has also been reported to influence growth-factor signalling and the gut–brain axis. Nearly all of this mechanistic work comes from rodent and in-vitro studies, and no validated human molecular target has been established.

Evidence

The BPC-157 literature is almost entirely preclinical: hundreds of rodent and in-vitro studies — the large majority from a single research group (Sikiric and colleagues) — report accelerated healing of tendon, muscle, ligament, bone, gut and other tissues. Independent replication is limited, and as of 2026 there are no completed, peer-reviewed controlled human efficacy trials. BPC-157 is not approved as a drug by the FDA, EMA or any comparable regulator; in 2023 the US FDA flagged it among substances that raise significant safety concerns for compounding. Any human use is unstudied.

Risks

Human safety has not been established in controlled trials: there are no robust data on long-term toxicity, immunogenicity or drug interactions in people. Because BPC-157 is pro-angiogenic, there is a theoretical concern about stimulating unwanted vascular growth (for example in an undiagnosed tumour), though this remains unproven in humans. In practice the dominant, concrete risk is product quality — gray-market 'research' vials vary in purity, may be underfilled or contain unverified salt forms, endotoxin or other impurities, and are used without any medical supervision.
